ApoB (apolipoprotein B)

Updated 2026-10-14Written for education by the Zenlo Health editorial teamHow we review
Quick answerApoB measures the number of atherogenic (plaque-forming) particles in your blood. Each LDL, VLDL, and Lp(a) particle carries one ApoB molecule, so ApoB counts particles, while LDL-C measures the cholesterol they carry.
Fasting and prepFasting is usually not required. Check your order for lab-specific instructions.
Typical turnaroundUsually 1 to 3 days. Some labs send it out, which can add time.
Also calledapo b, apolipoprotein b test

What it measures

Two people can share the same LDL-C and carry different numbers of particles. More particles mean more chances for one to lodge in an artery wall. ApoB helps most when triglycerides are high, when you have metabolic syndrome or diabetes, or when LDL looks fine but risk seems higher than expected.

Who should consider it

  • People with a family history of early heart disease
  • Anyone with diabetes, prediabetes, high triglycerides, or metabolic syndrome
  • People whose LDL looks normal but who want a closer risk read
  • Anyone on cholesterol treatment who wants a second marker of progress

How to read your result

ApoB (mg/dL)

The 2018 ACC/AHA guideline calls 130 mg/dL or higher a risk-enhancing factor. Targets for people at high risk are lower and personal. Lab ranges differ.

  • 130 mg/dL or higher is considered risk-enhancing in US guidance.
  • Treatment goals for people who already have heart disease are much lower than for the general population.
  • ApoB and LDL-C usually move together. A big gap between them is the reason to test.

Reference ranges differ between labs. Use the range printed on your own report.

When to get care right away. Chest pain or pressure, shortness of breath, fainting, or sudden weakness on one side: call 911. A blood test cannot rule out a heart attack.

What to do next

High ApoB is a reason to review lifestyle, blood pressure, blood sugar, and treatment options with a clinician. A single result is a starting point.

Questions people ask

Is ApoB better than LDL?

It answers a different question. LDL-C tells you how much cholesterol rides in LDL particles. ApoB tells you how many particles carry it. When they disagree, many cardiologists trust particle number more.

Do I need to fast for ApoB?

Usually no. ApoB changes very little after a meal. Follow your lab order if it says otherwise.

Can I order ApoB without a doctor?

Yes. Several online sellers offer it with an independent physician order, then you give a sample at a partner lab.
