Total estrogen

Written by the Zenlo Health editorial teamUpdated 2026-10-14Education onlyHow we review
Short answerA total estrogen test adds up the main estrogens in your blood (estradiol, estrone, and sometimes estriol) into one number. Most clinicians prefer a separate estradiol test, because it measures the most active estrogen directly and has clearer reference ranges.
Fasting and prep
No fasting needed. If you still have periods, note the cycle day of the draw. Follow the instructions on your lab order.
Turnaround
Usually 2 to 5 days. Some labs send it out, which adds time.
Also called
estrogen test, estrogen blood test, total estrogens, estrogen level

What it measures

The ovaries make most estrogen before menopause, and fat tissue and the adrenal glands make smaller amounts at every age. Levels swing across the menstrual cycle, drop after menopause, and rise in pregnancy. Birth control, hormone therapy, obesity, and liver disease all change the total.

Who should consider it

  • People whose clinician specifically asked for total estrogen
  • Anyone comparing results with an older total estrogen test
  • Most others: consider an estradiol test instead

How to read your result

  • Use the range printed on your report. Ranges depend on sex, age, cycle phase, and menopause status.
  • A single total number can't tell you which estrogen is high or low.
  • A normal total does not rule out a hormone problem, and an abnormal one does not diagnose one.

Reference ranges differ between labs. Use the range printed on your own report.

What to do next

If your result looks off, a clinician will usually order estradiol, FSH, and LH to sort out the cause.

Questions people ask

Should I get total estrogen or estradiol?

Estradiol in most cases. It measures the most active estrogen and gives a result clinicians can interpret more easily.

Do estrogen levels change during the month?

Yes, a lot. Levels rise before ovulation, dip, then rise again in the second half of the cycle.

Can I order an estrogen test without a doctor?

Yes. Online lab sellers offer total estrogen and estradiol with the order included.
